Application: Family Membership


For purposes of membership, a "Family" is two or more related people living in the same household, e.g., spouses/partners or parents and children. Unrelated persons, i.e., roommates, must make individual membership applications.


If there are more than four people in the family, additional members can be added on a second application.

CLUB MEMBERS' ASSUMPTION OF RISK, ACKNOWLEDGEMENTS & RELEASE OF LIABILITY

By our signatures below, each of us hereby acknowledge and agree to the following:

About the Club

Channel Islands Divers (“the Club”) is a group of people in and around Ventura County, California, who have come together voluntarily under the Club’s Constitution and By-Laws. The Club’s goals are to provide social, educational, and recreational opportunities related to the sport of scuba diving. Every year, the Club’s Board of four statutory officers and six other directors is elected by a majority vote of the current members. All Board members are volunteers and do not receive any payment for their work for the Club. The annual membership fees are used only to fund and support Club activities. These activities are chosen by a majority vote of the Board members in regular Board meetings, in accordance with the Club’s Constitution and By-Laws. Because Board members are volunteers and the Club is organized for specific purposes, California law protects the Club and its Board from legal action related to any activities they organize or oversee.


The Club does not carry insurance for itself, its officers and other directors, or its members. The Club and the Board do not guarantee the accuracy of information provided, the quality or safety of merchandise distributed at meetings or other activities, the skill or competence of any participant at a Club activity, or the safety of any Club activities.


Inherent Risks & Member Responsibilities

Activities like snorkeling, scuba diving, swimming, boat travel, and attending Club events can carry significant risks, including severe injury or death. Club activities may be far from medical services, decompression, recompression, or rescue services. Every participant is expected to take full personal responsibility for their scuba certification, their health and fitness to dive, equipment safety and knowledge for proper use, diving with the limits of experience and training, compliance with safety rules, and all their decisions during Club activities. Members are expected to follow the rules of any boats, dive operators or other facilities used during Club-sponsored excursions. While the Club encourages members to take all available precautions for safety, it does not regulate members’ behavior, equipment, or participation. The Club does not require members to list their certifying agency or number.


Member Assumption of Responsibility

By electronically signing this document, each of person named above ("each of us") acknowledge that we fully assume all risks of harm, including loss of life, personal injury, and property damage, involved in my participation in Club activities. Each of us do so voluntarily and have had an opportunity to ask questions. At least the initial member is eighteen years of age or older and legally competent to execute legal documents such as this one.

Release of Liability

As a condition of taking part in Club activities, each of us agree to hold harmless and release the Club from liability for any harm that may occur to any of us during a Club-sponsored activity. Each of us also agree to hold harmless and release the Club’s officers and members from responsibility for any harm that may occur to any of us during a Club-sponsored activity, unless due to intentional, reckless, and deliberate misconduct by a member or an officer. Each of us understand and agree that this explicit assumption of risk and release of liability will apply to our heirs, assigns, representatives, executors, spouse, and agents.


In recognizing each of our explicit assumption of risk and agreeing to the above release of liability, each of us are not relying on any written or spoken statements by anyone, any Club officer or member, claiming to change or waive the terms of this acknowledgement. Each of us fully understands that this document means exactly what it explicitly states.


Each of us agree that, if there is any disagreement about the terms, intentions or implications of this document, that disagreement will be settled using California’s laws, no matter where anyone involved lives or where they come from. If any part of this agreement is not valid or cannot be enforced, that part will be removed but the rest of the agreement will still apply.


Permission to Use Images, Video, Audio and Name

Each of us gives our full permission to the Club to use images, video or other records of us and our participation in Club activities, including our name(s), likenesss and voices. Parents give full permission on behalf of minor childern.


Ending Membership & Revoking Agreement

We understand that this agreement can be revoked and our membership ended at any time except during a Club activity. We understand that to do so, one of us must immediately send written notice to the Club’s President (e.g., via an email to president@channelislandsdivers.org), cancelling our Club membership. We understand that annual membership fees are non-refundable and that we will not be able to participate in Club activities unless I rejoin.
